Tirzepatide dosage chart in units

Units to draw = dose (mg) ÷ concentration (mg/mL) × 100. A 5 mg dose is 50 units from a 10 mg/mL vial but 25 units from a 20 mg/mL vial, so find the column that matches your vial's label.

Use your own vial's concentration. The same dose can be 12.5 or 50 units depending on the vial. Your prescriber sets the dose; this chart only converts it. If your pharmacy gave you a dosing chart, follow theirs and use this one to double-check.

Label dosing schedule

This is the titration in the FDA labels for Zepbound (weight management) and Mounjaro (type 2 diabetes). Both are injected once a week.

Units to draw for each dose

Find your vial's concentration across the top and your dose down the side. Values are units on a U-100 insulin syringe (100 units = 1 mL).

Greyed values are over 100 units (1 mL) and won't fit in one U-100 syringe. That dose isn't used with that vial strength; check your label.

Find your vial's concentration

Concentration is the amount of tirzepatide in each mL of liquid. If your label lists mg and mL separately, divide them:

Any other combination: use the tirzepatide units calculator, which accepts either the mg/mL or the vial amount and liquid volume.

Reading the result on a syringe

Some answers fall between the lines, such as 16.7 units for 2.5 mg at 15 mg/mL. A 0.3 mL syringe is marked every unit; a 1 mL syringe every 2 units. Use the smallest syringe that holds the dose, and ask your pharmacist how to round when the number isn't on a line. The insulin syringe guide shows the markings on each size.

Why charts online disagree

Most tirzepatide charts you'll find are handouts from a single clinic, written for the one vial strength that clinic dispenses. Used with a different vial they give the wrong number of units, even though the mg doses are identical. That's why this chart lists units by concentration instead of one column.

Questions

How many units is 2.5 mg of tirzepatide?

25 units at 10 mg/mL, 50 units at 5 mg/mL, about 16.7 units at 15 mg/mL and 12.5 units at 20 mg/mL. Check the concentration on your vial before using any chart.

How many units is 5 mg of tirzepatide?

50 units at 10 mg/mL, about 33.3 units at 15 mg/mL, about 29.4 units at 17 mg/mL and 25 units at 20 mg/mL.

What concentration is a 30 mg tirzepatide vial?

It depends on how much liquid is in it. 30 mg in 3 mL is 10 mg/mL; 30 mg in 2 mL is 15 mg/mL. Your pharmacy label should state the mg/mL. If it doesn't, ask before using the vial.

What is the starting dose of tirzepatide?

The Zepbound and Mounjaro labels start at 2.5 mg once weekly for 4 weeks. That dose is for starting treatment, not a maintenance dose. Your prescriber decides when and how to increase it.

Can I use this chart for Zepbound pens?

No. Pens are pre-set to a single dose and don't use syringe units. The units columns only apply when you draw from a vial with a U-100 syringe.
